Important Notice To Licensed Veterinarians And Trainers

Please see the Veterinary Clearance Certificate template we would like to ask all veterinarians to use when providing veterinary clearance notifications to Racing NSW.

The document is an interactive PDF that allows you to type directly onto it. If you prefer, you can also choose to print it out and complete it by hand.

We consider it important for the veterinarian to be aware/document the reason for the veterinary clearance notification/examination; as such, we ask that you state this reason in the area provided under the statement, “I certify I have recently (within 24 hours) examined the above-named horse for the below recorded injury, illness or condition”.

If you wish/need to provide any supportive documents, please provide these as attachments together with the Veterinary Clearance Certificate.
